Registered Nurse (RN) - Remote Opportunity for Telehealth Nurse Navigator!POSITION SUMMARY:Under general direction of the Senior Nurse Manager or Nurse Manager, the Telehealth Nurse Educator (Patient Single Point of Contact) is responsible for serving as the patient and/or caregiver single point of contact for all interactions including clinical and reimbursement. This position will serve as the dedicated voice to the patient for product specific education, care coordination, and individualized adherence support outreach. In addition, the position will support on-going adherence by working with the patient on short and long term goals using care planning tools. Serving as the dedicated voice to the patient and/or caregiver, this position will also communicate the results of the benefits investigation, enroll and administer co-pay, patient assistance, and clinical trial, collaborate on shipment of free goods, conduct missing information outreach to patients and provide on-going commercial service support for the patient.PRIMARY DUTIES AND RESPONSIBILITIES:Provide adherence services focused on primary non-adherence and in-home injection trainingCoordinate in home injection training support to cover techniques, review of injection materials, supplies, appropriate disposal and continued patient follow upProvide consumer support in a call center environment for patients receiving treatment.Risk assess patient for compliance and adherence to treatment regimenIdentify barriers to careSupport patient care planning and development of short and long term goalsProvide information about disease processProvide emotional support to patient and family membersAccepts inbound calls and makes outbound calls to enrolled patientsReceives notification through, but not limited to: web portal, live chat, patient assistance program, fax enrollment form, electronic enrollment form or phone; calls patient to provide information about the product, program, send out information kits if needed and management advise for any side effectsEnter all consumer activities accurately in clinical data baseIntakes and reports adverse events as directedProvide case management supportProvide patient support for payer policies and coverage issuesCommunicate reimbursement and financial assistance informationConduct missing information outreachCommunicate completed benefits investigationsIdentify, enroll, and administer the co-pay programResponsible for administration of the PAP and clinical trial programsConduct on-going assessment of eligibility for commercial servicesCollaborate with internal and external stakeholders to order, coordinate, and confirm shipmentsMust be able to work a flexible schedule and have flexibility for changing program needsPerforms related duties as assignedQualificationsEXPERIENCE AND EDUCATIONAL REQUIREMENTS:Must possess a current license issued by a state or states or jurisdiction(s) within the United States, and be unrestricted in any state with the ability to obtain licensure in all 50 statesGraduation from an accredited nursing programExperience with and/or potential to provide high quality customer service3 years nursing experience requiredTelephonic experience a plusCertification may be requiredGraduation from an accredited nursing programCase management experience preferred MINIMUM SKILLS, KNOWLEDGE AND ABILITY REQUIREMENTS:Excellent oral and written communication skillsExcellent interpersonal skillsAbility to use a computer for data collection proficientlyStrong organizational skills; attention to detailGood critical thinking skillsStrong computer skills Equal Opportunity Employer/Minority/Female/Disability/Veteran
